Admissions Information
Patient Review Instrument
Prior to admission, our trained professionals will meet with you to complete a Patient Review Instrument or P.R.I. This form, developed by New York State, is a medical, psychological and social evaluation that provides an overall picture of your needs and current level of functioning. It also contains information regarding a diagnosis, recent therapies and diet.

Admissions Application Form
You will need to complete our Admissions Application Form that includes confidential questions pertaining to your personal and financial history. Be assured these forms remain strictly confidential and are utilized for admissions procedures only. Together, the P.R.I. and Admissions Application Form help our clinical team accurately determine the type and level of care that will best suit your needs.
After your P.R.I. and application are reviewed, you will be contacted based on room or bed availability. Whether you are being admitted to the facility from your home or from the hospital, our caring staff will work closely with your Primary Care Physician and/or the hospital Discharge Planner to coordinate your arrival.

Insurance
Our facilities are participants with Medicare, all major Medicare replacement HMO’s (Univera, Encompass 65, Senior Blue, etc.) and Medicaid. Our Admissions Coordinators will be happy to help determine your eligibility for either program. If you are ineligible for Medicare or Medicaid and paying privately for your care, we will provide you with information on our rate structure.

Primary Care
While you are a resident at our facility, we offer two choices for your primary care needs:
  1. You may choose one of our highly qualified staff Physicians or Physiatrists to be responsible for your care.
  2. You may choose to have your Primary Care Physician attend to your health needs.
We would be happy to credential your own Physician, if they are interested in tending to your needs at the facility.
In addition, our staff prides itself on providing superior 24-hour nursing and personal care. We will assist you with medications, ambulation, transferring (i.e. moving from a bed to a chair), meals and activities of daily living. We can also provide you with walkers, wheelchairs, canes and other devices as recommended by our Physical Therapists.

Upon Admission
When entering our home-like facilities, you will discover that each room is already fully furnished. Since this will be your new home, we encourage you to bring any personal items you have that will make your room most comfortable. You are more than welcome to bring furniture, pictures, a television and other personal belongings to accent your living area.
You will meet with our Registered Dietician and Dietary staff to identify your food preferences and any special diet that your Physician has prescribed.
You will be given a schedule of activities designed by our professional Activity staff. They will coordinate a variety of daily programs geared toward your individual needs and interests. Many of these include cooking, crafts, BINGO and inter-generational events. Weekly religious services are also held to fulfill the spiritual needs of our residents.

Visitors
Visitors are welcome and encouraged at our facilities. After all, this is your home! We have areas available for family gatherings and celebrations, along with more private meeting rooms. Your family and friends are encouraged to visit as often as they like.
