Subacute Admissions Assessment
Prior to your admission into one of our Subacute Rehabilitation Centers, you will meet with a trained professional to complete a Subacute Admissions Assessment. This form helps our clinical team determine the most appropriate type of rehabilitation necessary to meet your specific needs. If circumstances allow, we encourage you to pre-plan your rehabilitative stay and, in all cases, work closely with the hospital Discharge Planner and your Primary Care Physician to ensure a smooth transition into our facility.
While your discharge date is set prior to or at the time of admission, your length of stay will depend on the nature of your illness. Our team of professionals will meet with you often to evaluate your status and begin post-discharge planning. In addition, the amount and type of therapy you will receive will also depend on your medical status. Our therapy services are available twice a day, up to seven days a week at most of our facilities. Our Therapists and your Physician will work closely to schedule the amount of therapy that is appropriate for you.
Insurance
Our facilities are participants with Medicare, all major Medicare replacement HMOs (Univera, Encompass 65, Senior Blue, etc.) and Medicaid. Our Admissions Coordinators will be happy to help determine your eligibility for either program. If you are ineligible for Medicare or Medicaid and paying privately for your care, we will provide you with information on our rate structure.
Primary Care
While you are a patient at our facility, we offer two choices for your primary care needs:
- You may choose one of our highly qualified staff Physicians or Physiatrists to be responsible for your care.
- You may choose to have your Primary Care Physician attend to your health needs.
We would be happy to credential your own Physician, if they are interested in tending to your needs at the facility.
In addition, our staff prides itself on providing superior 24-hour nursing and personal care. We will assist you with medications, ambulation, transferring (i.e. moving from a bed to a chair), meals and activities of daily living. We can also provide you with walkers, wheelchairs, canes and other devices as recommended by our Physical Therapists.
Upon Admission
When entering our home-like facilities, you will discover that each room is already fully furnished. Since this will be your temporary home, we encourage you to bring any personal items you have that will make your room most comfortable. You are more than welcome to bring pictures, and other personal belongings to accent your living area.
You will meet with our Registered Dietician and Dietary staff to identify your food preferences and any special diet that your Physician has prescribed.
You will be given a schedule of activities designed by our professional Activity staff. When you are not completing your therapy regimens, you may enjoy a variety of organized activities or simply relax and enjoy your free time. Many of the activities include cooking, crafts, BINGO and inter-generational events. Weekly religious services are also held to fulfill the spiritual needs of our patients.
Visitors
Visitors are welcome and encouraged at our facilities. We have areas available for family gatherings and celebrations, along with more private meeting rooms. Your family and friends are encouraged to visit as often as they like.
