Subacute Rehabilitation


     Our staff is pleased to offer state-of-the-art subacute programs for individuals requiring rehabilitative therapy between hospital and home. The concept of Subacute Rehabilitation has developed at a time when Americans are living longer, healthier lives, reducing the need for continuous nursing care after an illness or injury.

     The McGuire Rehabilitation Centers have been designated specifically for short-term stays and feature an all-nurse staff working in concert with a Physiatrist, Medical Directors, Physical Therapists, Registered Occupational Therapists and other specialists trained in all aspects of physical, emotional and medical rehabilitation. Our individually tailored therapy and nutrition regimens focus on maximizing a patient’s ability to lead an independent life. Our social workers and case managers work with the patient and family to ensure a safe and appropriate discharge back into the community.

Our state-of-the-art rehabilitative programs include the following:

Cardiac Care
This program combines skilled nursing care and medical treatments to assist in the prevention of complications and improve the rehabilitation of patients with cardiac conditions. A clinical team including Nurses, Therapists, Pharmacists and Social Workers provides a specially designed program for cardiac care. Individuals with heart disease, congestive heart failure and those who are ready to leave the hospital after surgery or a heart attack are perfect candidates for this program.
Neurologic Rehabilitation
The goals of our program focus on helping individuals recover the mental and/or physical functioning affected by a neurological condition. Physical, Occupational and Speech Therapies are emphasized through an individualized plan of treatment designed by our clinical team. Patients are able to receive skilled nursing care while undergoing therapy programs for accomplishing daily activities. Individuals who have suffered strokes, paralysis, loss of memory, mobility or speech and the inability to eat are ideal for this program.
Orthopedic Rehabilitation
This program meets the needs of patients who have had orthopedic surgery or suffered an injury to the back, arms or legs. Patients undergo a combination of treatment regimens, as well as physical and occupational therapies, designed to return them to the highest level of functioning as soon as possible. All care is administered under the direction of a board-certified Physiatrist who works in conjunction with the patient’s Primary Care Physician and Surgeon. Individuals who have had joint replacements, hip or leg fractures, motor vehicle accidents, spinal injuries, amputations or osteoporosis are ideal candidates for this program.
Oncology Management
Often times, individuals with cancer require extensive intervention and care that cannot be provided in their home. Our program has been designed specifically to meet the needs of these patients and includes the administration of treatments, supportive care and pain and symptom management. This form of care can be utilized to assist patients in recovering from extensive surgery, for reconditioning during chemotherapy and radiation and to promote comfort and minimize complications during the end stages of the disease.
Post-Surgical Care
Patients requiring a combination of intravenous medications, wound care, nutritional support, skilled nursing care and physical therapy will benefit from this type of care. Common surgical procedures that indicate a potential need for Subacute care include cardiac bypass, amputations, arterial grafts, bowel resections, colostomies, organ transplants, radical prostatectomies and others.
Pulmonary Rehabilitation
This program allows individuals with respiratory ailments and pulmonary disease to obtain specialized care outside of the hospital setting. The objectives of our program focus on having patients discharged from the hospital sooner, avoid infections and complications, obtain more intensive and individualized care and return home at a higher level of functioning. Patients are able to receive oxygen and respiratory therapies, non-invasive ventilation, intravenous medications, skilled nursing care and closely monitored exercise programs. In addition, a clinical team works closely with the patients’ Primary Physicians to ensure their care after returning home. All respiratory-related patients who have completed the acute phase of their illness, but are not yet ready or able to return home, are excellent candidates for this program.

Utilizing this measurement, our facilities have proudly scored significantly higher than other regional providers in patient improvement, discharge to home and cost effectiveness.
