Brookhaven Health Care Facility in East Patchogue is pleased to announce that it has been listed in the top quintile in New York State’s quality pool metrics and received the highest score among all Suffolk County facilities.

The state’s report, which was distributed in January, evaluates nearly 600 facilities based on measurable indicators of quality including such metrics as: pressure ulcers, falls, catheter use, deterioration in activities of daily living, flu vaccine rates, resident weight loss, rehospitalization rates and others.

Brookhaven Health Care Facility features 160 beds with services including subacute and outpatient rehabilitation, skilled nursing care and respite/short-term care. In addition, Brookhaven Health Care Facility’s sister facilities in Western New York all ranked in the top quintile with Autumn View Health Care Facility in Hamburg earning the top score among all Western New York facilities.

Information for the state rankings is calculated and managed by the New York State Department of Health.

“Our performance in the state’s ranking system highlights the culture of quality that exists at every level within our facilities,” stated Administrator Debi Gaines. “We’re focused on creating healthier communities by meeting a full spectrum of needs from short-term rehabilitative stays to 24-hour care.”

Brookhaven Health Care Facility is proud to be affiliated with Brookhaven Memorial Hospital Medical Center for high quality rehabilitative care. More information can be found at www.brookhavenhealthcarefacility.com or www.medicare.gov.
